Health & Safety: Accident and Near Miss Reporting

The safety of our members and visitors is a priority. To make sure we meet our responsibilities and keep the site safe, all accidents and near misses must be recorded.

• Accident Form – complete if an incident causes injury or damage.

• Near Miss Form – complete if something could have caused harm, even if no one was injured.

Reporting helps us learn from incidents, take action to prevent them happening again, and ensures we meet insurance requirements.
